1. Acceptance of these terms

By accessing or using insurancequotebase.com (the “Site”), you agree to be bound by these Terms & Conditions and by our Privacy Policy. If you do not agree, please do not use the Site.

2. What we provide

InsuranceQuoteBase LLC operates an insurance referral and comparison service. We help you compare health insurance plans and connect you with licensed insurance agents who can explain your options and, if you choose, assist you with enrollment.

We are not an insurance carrier. We do not underwrite policies, set premiums, determine eligibility, approve applications, or pay claims. Those decisions rest entirely with the insurance carrier you apply to.

InsuranceQuoteBase LLC is a private entity and is not affiliated with, endorsed by, or sponsored by the U.S. government, the federal Medicare program, the Centers for Medicare & Medicaid Services (CMS), or the Health Insurance Marketplace.

3. This is not professional advice

The content on this Site is general information for educational purposes. It is not individualised insurance, legal, tax, financial or medical advice, and it does not account for your particular circumstances.

Insurance rules, plan benefits, provider networks, drug formularies, premiums and enrollment deadlines change regularly and vary by state and by carrier. Always verify anything time-sensitive or consequential with a licensed agent, the carrier directly, or an official source such as Medicare.gov before acting on it.

4. Eligibility

To use this Site and submit a quote request you must:

• Be at least 18 years of age;
• Be a resident of the United States;
• Provide information about yourself, or about someone for whom you have legal authority to act.

5. Your obligations

You agree to:

• Provide accurate, current and complete information;
• Not submit another person’s information without their knowledge and permission;
• Not use the Site for any unlawful purpose, or to submit fraudulent, automated or abusive requests;
• Not attempt to gain unauthorised access to any part of the Site, its systems or its data;
• Not scrape, harvest, reverse engineer or use automated means to extract content or data from the Site.

Providing inaccurate information may result in an inaccurate quote, and may affect a carrier’s decision on any application you submit.

6. Electronic communications and contact consent

By providing your telephone number and checking the contact consent box, you give prior express written consent to receive calls and text messages as described in that disclosure and in our Privacy Policy. Consent is not a condition of purchase, and you may revoke it at any time by replying STOP, telling an agent, or contacting us.

I consent to receive required notices, disclosures and policy documents electronically, and I confirm I am able to access and retain information sent by email.

7. No guarantee of coverage, savings or availability

Submitting a quote request does not enroll you in any plan, does not create an insurance contract, and does not affect any coverage you already have. Requesting a quote places you under no obligation to enrol in any plan. Plan availability, benefits, premiums, copayments, coinsurance and provider networks vary by carrier, plan and geographic location, and are subject to change.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE (TTY users should call 1-877-486-2048), 24 hours a day, 7 days a week, or consult your local State Health Insurance Assistance Program (SHIP) to get information on all of your options.

Eligibility for premium tax credits and cost-sharing reductions depends on household income, household size and other factors, and is determined by the Health Insurance Marketplace, not by us. Any savings figures shown on this site are illustrative estimates only.

Any figures, estimates or examples shown on the Site are illustrative. Your actual premium, benefits and out-of-pocket costs are determined solely by the carrier and the specific plan you enroll in.

8. Third-party carriers, agents and links

We may connect you with third-party licensed agents and carriers. While we take care in choosing partners, we do not control them and are not responsible for their acts, omissions, advice, products, privacy practices or the outcome of any application.

The Site may link to third-party websites, including government sites. Those links are provided for convenience. We do not control and are not responsible for the content or practices of any linked site.

9. Intellectual property

The Site, its design, text, graphics, logos and compilation of content are owned by InsuranceQuoteBase LLC or its licensors and are protected by copyright, trademark and other laws. You may view and print pages for your own personal, non-commercial use. Any other reproduction, distribution, modification or commercial use requires our prior written permission.

10. Disclaimer of warranties

THE SITE AND ALL CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE AND NON-INFRINGEMENT.

We do not warrant that the Site will be uninterrupted, timely, secure or error-free, or that any information on it is complete, accurate or current.

11. Limitation of liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, INSURANCEQUOTEBASE LLC AND ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY OR PUNITIVE DAMAGES, OR FOR ANY LOSS OF PROFITS, DATA, GOODWILL OR COVERAGE, ARISING OUT OF OR RELATING TO YOUR USE OF THE SITE, WHETHER BASED IN CONTRACT, TORT, NEGLIGENCE, STRICT LIABILITY OR OTHERWISE, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

Some jurisdictions do not allow the exclusion or limitation of certain damages, so some of the above may not apply to you.

You agree to indemnify and hold harmless InsuranceQuoteBase LLC and its officers, directors, employees and agents from any claim, demand, loss or expense, including reasonable legal fees, arising out of your breach of these terms, your misuse of the Site, or your submission of another person’s information without authority.

13. Governing law and disputes

These terms are governed by the laws of the State of CT, without regard to its conflict-of-laws rules. You agree that any dispute will be brought in the state or federal courts located in New Haven, CT, and you consent to their jurisdiction.

Note for review: many insurance marketing businesses include a binding arbitration clause and a class-action waiver here. Whether to do so, and whether such a clause is enforceable in your states of operation, is a decision for your counsel. No arbitration clause is included in this template.

We may revise these terms at any time. The “last updated” date and version at the top of this page indicate the current revision. Your continued use of the Site after a change constitutes acceptance of the revised terms.
